Hemophilia, is the oldest known inherited bleeding disorder has been widely recognized through the ages mainly because of its association with the royal families of Europe. The disease, once dreaded as bleeding disorder has long since ceased to be synonymous with misfortune and doom. The immense resourcefulness and perseverance displayed by researchers worldwide has meant continuously evolving treatment modalities and near normal life spans for Hemophiliacs today. Even though a complete cure still eludes us, modern medicine has very effective systems in place to manage and effectively treat Hemophilia.

The incidence of hemophilia worldwide is 1/10,000. It is estimated that in India, with population more than 1000 million, there should be at least over 100,000 hemophilia A patients in the country and at least 50,000 persons with severe hemophilia.1 Of these, less than 15% of estimated number have been diagnosed.1 The data available with Hemophilia Federation of India gives the number of people with hemophilia who receive treatment as 12,500.2
The awareness levels are remarkably low. Hospital data indicate that patients in India suffer from preventable morbidity because doctors do not get adequate exposure in the context of suboptimal availability of infrastructure for diagnosis or treatment of hemophiliac. This is also one of the major reasons for the majority of cases remaining undiagnosed. Several states Govt such as Tamil Nadu, Delhi, UP, Karnataka, Bihar, Assam are already supporting the treatment of Hemophilia.

Highly purified factor VIII and IX concentrates are the safest and pure treatment option. Factor concentrates have been employed extensively to manage Hemophilia and to deal effectively with bleeding episodes. Current processes for manufacturing hemophilia treatment products, when well managed, can produce products with very low risk of viral transmission. However, because hemophilia treatment products sourced from human blood have a well-established history of transmission of blood-borne infectious agents, such as HIV and hepatitis; it is very important to ensure that products being considered for purchase are safe and free from viral infection.

The pathogen safety issue has taken its toll in India. About 10-14% of PWH in India have hepatitis B infection and 24-30% of them are positive for hepatitis C.3 These viral transmissions are unlikely to reduce without universal use of high purity factor concentrates or recombinant products for hemophilia care.

Since a past few years the Tamil Nadu govt (DME, Medical Colleges, TNMSC) has been supporting treatment of Hemophilia by allocating funds and providing Anti Hemophilia Factors to all patients suffering with bleeding disorder (which may lead to joint damage and fatal in few severe cases). The patients have been using these high purity factors (provided by Govt since last 3 years; and previously through the Hemophilia society and/or manufacturers) with no adverse event reported.

Doctors and treating clinicians have been providing these factors through 12 Medical colleges across TN. Industry is also supporting through Continued Medical Education ensuring improved treatment regimen and following of prescribed protocols of treatment. Hemophilia chapters representing patients suffering with Hemophilia are working tirelessly to improve awareness of availability of factors and educating fellow patients.

There have been robust tender process and selection criterion to select the best product, with highest purity of Factor, approved by US FDA, ensuring safety to patients. Due to this patients are confident and would not want the quality of the product made available at the various government hospitals and by service providers to be compromised as it may lead to deterioration in their health and other complications. They are expecting that the government will take a serious call and strike a balance between quality, availability and affordability.

Nowadays certain generic versions with lower purity of factors are also available in the market with no track record, limited clinical efficacy and limited supply capabilities and lower pricing which may have a diverse effect on the patients and may also result in blood infections etc.

Mr Parthasarathy, Vice President, Hemophilia Society Madras Chapter said “It is the responsibility of the government to provide the best of treatment to patients especially when life threatening disorders like Hemophilia is concerned. Till date, patients have been receiving safe clotting factors by government and our concern is that they will continue to support patients with same product and not succumbing to inferior quality product.”

There are a number of Hemophilia patients who have suffered grossly as they were administered poor quality factors in some other hospitals. Hence the need of the hour is to take a step – by – step approach focusing mainly on spreading awareness, establishing basic laboratories, training technical personnel and most importantly making the Factor VIII concentrates available that has been passionately advocated by needy patients and advocacy groups. Over the years Hemophilia chapters representing patients suffering with Hemophilia are working tirelessly to improve awareness of availability of factors and educating fellow patients on choosing the right factor.

Treatment for Hemophilia patients have been made available by the government of TN to all patients since past few years and they will hopefully continue to do so by providing good quality products to all Hemophilia patients. Indians are amongst the fortunate citizens that the government is so considerate and helpful towards patients who suffer from severe diseases like Hemophilia.

When asked about how the TN Govt has been supporting Hemophilia by allocating funds and providing high quality anti Hemophilia Factors to all patients suffering from this bleeding disorder which has considerably improved the well-being of Hemophiliacs in the state. Dr. Gunasekaran, Medical Superintend, Royapet Medical College Hospital said, “Yes, they are being taken care and are doing well”.

GOLLAPUDI SRINIVAS NATIONAL AWARD 2011 goes to “MADHAV RAMADASAN” for his Debut film “MELVILASOM” (Malayalam)

The 15th Gollapudi Srinivas National Award for the Best debut film was announced by Gollapudi Srinivas Memorial Foundation for the year 2011. The Award goes to Mr Madhav Ramadasan for his film “MELVILASOM”(Malayalam).

‘Melvilasom’ ( The address),is an astonishingly gripping court room drama narrated in real time with no frills, and no distractions It is a brilliant narrative of a closed-door army court martial starkly told. The military discipline does not camouflage the undercurrents of bravery and betrayal. For a debut director, he has effectively displayed his control over the art of film-making in this hundred-minute film, terse and brutally concise. The monotony that could creep in such mono-chromatic narrative has been skillfully avoided Truly a just story honestly and competently told .Cinema is generally perceived as larger-than-life presentation with ample scope for glamourous stars and exotic locations. Madhav Ramadasan in his maiden attempt has rewritten the dynamics of film making with just six characters and no women incredibly shot within a set. The director has found the right address in ‘Melvilasom’. This ‘address’ has fetched him this prestigious award and is likely to bring in many more..

The film was selected from 23 nominations from across the country in many languages including Hindi, Malayalam, Bengali, English, Tamil, Marathi, Assamese and Kannada. The jury consisted of Sri Singeetam Srinivasa Rao, Sri Krishnan of Kavithalaya and Ad Film Maker Smt. Latha Menon. This is the 15th year in a row, where the Best Indian Directorial Talent in feature films is being recognized by the Chennai based foundation.
